What it Does: The company has created a free, secure app by which patients can send and request their family's health records -- including immunizations,  medical, dental and vision, as well as prescription and even over-the-counter medications, lab and imaging -- through their smartphone. Users can upload data to the app as well.

For hospitals, the company's analytics allows them to gain insights about patient engagement, something that has become increasingly important as the industry transitions to a value-based platform. ChartSpan recently raised $3.2 million from a syndicate of Texas investors, according to MobiHealth News.
